$26.77 - $34.80The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
GENERAL PURPOSE: In an inpatient setting, remains accountable for provision of direct care to assigned patients. Contributes to unit activities and supports enhancement of the care delivery system. Works as a team member and assists other staff members with implementing patient care. Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD. Participates in the plan of care, implements appropriate nursing interventions, and contributes to patient/family education through collection of data, interdisciplinary team collaboration, documentation and excellent communication.FT positions available. Primary 12 hour day shifts, E/N and 8 hour shifts may be required. Holidays and weekends per unit policy.

Patients enrolled in Hospital at Home receive inpatient hospital level care in the comfort of their own homes, rather than in a traditional hospital unit. Joining the Hospital at Home team is a rare opportunity to be part of expanding a groundbreaking care model. This program is newly launched and is looking for experienced, forward-thinking and creative LPNs to join. The LPN will work in the assistive role with primary functions of assisting with admissions, discharges, remote patient monitoring, care coordination and logistics management. Minimum 2 years of recent acute care, ICU or ED experience required. 5+ years of broad clinical expertise is preferred. This position requires a valid NYS driver’s license and reliable transportation.
