Our client, a respected provider of home healthcare and senior support services, is seeking a compassionate and highly organized Remote Senior Care Coordinator to manage patient care plans and provide essential support to seniors and their families. This is a fully remote position, allowing you to make a significant impact from anywhere in the United States. As a Senior Care Coordinator, you will be the primary point of contact for clients, assessing their needs, developing personalized care plans, and coordinating a wide range of services, including in-home care, medical appointments, transportation, and community resources. You will work closely with a team of caregivers, healthcare professionals, and external agencies to ensure seamless and high-quality care delivery. Key responsibilities include conducting initial client assessments (via video conferencing), creating detailed care plans, scheduling caregiver visits, monitoring patient progress, and communicating regularly with clients and their families to ensure satisfaction and address any concerns. The ideal candidate possesses a strong understanding of geriatric care, common health conditions affecting seniors, and available community support systems. Exceptional interpersonal, communication, and problem-solving skills are essential, as is a compassionate and empathetic approach. Proficiency with electronic health record (EHR) systems or client management software is required. You must be adept at managing multiple cases simultaneously, prioritizing tasks, and maintaining accurate and confidential client records. Develop, implement, and manage personalized care plans for clients. Coordinate in-home care services, including scheduling caregivers and monitoring their performance. Liaise with healthcare providers, specialists, and other community resources to facilitate comprehensive care. Serve as the primary point of contact for clients and their families, addressing inquiries and concerns promptly. Monitor client progress and well-being, making adjustments to care plans as needed. Maintain accurate and confidential client records in the designated system. Ensure compliance with company policies, procedures, and relevant regulations. Educate clients and families about available services and support options. Advocate for clients' needs and promote their independence and quality of life. Qualifications: Bachelor's degree in Social Work, Nursing, Gerontology, Healthcare Administration, or a related field. Minimum of 3-5 years of experience in case management, social work, or healthcare coordination, preferably with a focus on geriatrics or senior care. Strong understanding of senior care needs, chronic conditions, and community resources. Excellent communication, interpersonal, and active listening skills. Proficiency with client management software, EHR systems, or similar platforms. Strong organizational and time management abilities, with the capacity to manage multiple caseloads. Empathetic, compassionate, and patient demeanor. Ability to work independently and collaborate effectively in a remote team setting.
