Develop individualized care plans in collaboration with clients and their families. Coordinate and facilitate access to essential services, including housing, healthcare, employment assistance, mental health support, and legal aid. Provide ongoing support, counseling, and advocacy to clients. Monitor client progress and regularly review/update care plans as needed.
