Behavioral Health Social Work Case Manager
Job Description:
- Support and coordinate the mental and physical health care needs of patients on an assigned caseload
- Screen and assess patients for common mental health and substance abuse disorders and facilitate patient engagement and follow-up care
- Provide brief behavioral interventions using evidence-based techniques
- Track patient follow up and clinical outcomes using a registry
- Participate in regularly scheduled caseload consultation with the psychiatric consultant
- Support psychotropic medication management as prescribed by medical providers
- Facilitate coordination of care to available resources as needed and provide social work case management
Requirements:
- Education: Licensed Clinical Social worker, Licensed Professional counselor or Licensed in a related field of study
- Completion of degree and all post-degree supervision for licensure and at least 2 years of post-degree experience
- Experience of successfully delivering evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders
- Ability to effectively engage patients in a relatively short-term therapeutic relationship, via video and/or audio
- Evidence of high degree of organizational, collaborative and communication skills
- Licenses: Licensed in one or more states. Willingness, education and experience sufficient to obtain multiple state licenses.
