Registered Nurse Oncology Navigator
To support high-acuity members navigating their cancer journey, the full-time Complex Oncology Nurse Navigator will conduct clinical assessments, coordinate care transitions, and educate patients and caregivers remotely while ensuring culturally competent care and improving service offerings through feedback communication. Key responsibilities Conduct telephonic clinical assessments and monitor health changes for oncology patients Coordinate care management and discharge plans with healthcare teams and providers Build strong relationships with members, caregivers, and providers to optimize care and support social determinants of health Required qualifications Active, unrestricted compact Registered Nurse (RN) license and willingness to obtain additional state licenses Degree in Nursing (ADN or BSN) required; BSN preferred At least 5 years of nursing experience with 3 years in high-acuity adult oncology Certification as an Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) Experience with technology, including electronic health records and video conferencing tools Apply To This Job
