Job Title: Lead Care Coordinator Location: Remote Employment Type: Full time Compensation: $82,500 Role: The Lead Care Coordinator is a strategic people leader, operational problem-solver, and technology champion responsible for scaling and optimizing FamilyWell's Care Coordination function. This role oversees the performance, development, and day-to-day operations of the Care Coordinator team while leading initiatives that leverage technology, automation, and process improvement to scale operations efficiently. The ideal candidate is passionate about developing high-performing teams, using data to drive decision-making, and creating innovative systems that allow Care Coordinators to focus on high-value patient and provider interactions while technology and automation reduce administrative burden. Reporting to the Manager of Clinical Operations, this individual will partner closely with Product, Clinical Operations, and Clinical Leadership to identify opportunities for automation, develop scalable workflows, improve operational efficiency, and ensure seamless coordination across the patient journey. This role will serve as a critical bridge between frontline operations and product development, helping shape technology solutions that improve team performance, patient outcomes, and provider experience The Lead Care Coordinator is accountable for both team performance and operational innovation, ensuring Care Coordinators consistently achieve key performance metrics while leveraging technology and automation to maximize patient-facing impact.

Key Responsibilities

Team Leadership & Development: Lead, coach, and develop a team of Care Coordinators through regular supervision, performance management, mentorship, and professional development. Foster accountability, collaboration, and operational excellence. Care Coordination Operations: Maintain a reduced Care Coordinator caseload to stay connected to frontline workflows, patient experience, and clinic partner needs while serving as an escalation resource for complex coordination issues. Technology & Automation Strategy: Identify, prioritize, and implement opportunities to automate administrative work across the Care Coordination function. Continuously evaluate tools, systems, AI solutions, and workflow enhancements that improve efficiency and scalability. Product Development Partnership: Act as the primary operational stakeholder for Care Coordination workflows. Partner closely with the Product team to design, test, and refine technology solutions that improve team performance, patient experience, provider experience, and operational outcomes. Workflow Design & Optimization: Analyze existing processes, identify bottlenecks, and develop scalable workflows that reduce manual effort, eliminate inefficiencies, and support organizational growth. Data-Driven Decision Making: Monitor operational metrics, team performance, patient outcomes, and workflow efficiency. Use data to identify trends, inform strategic decisions, prioritize improvements, and measure the impact of operational changes. Operational Problem Solving: Proactively identify operational challenges, investigate root causes, develop solutions, and drive implementation through completion. Closing the Loop: Ensure timely follow-up and resolution of operational issues, patient concerns, clinic partner requests, and internal process gaps. Create systems that promote accountability and seamless communication across teams. Innovation Leadership: Champion a culture of continuous improvement, experimentation, and innovation. Stay informed about emerging healthcare technology, automation tools, AI capabilities, and operational best practices. Scaling Operations: Build systems and processes that enable the Care Coordination function to scale efficiently while maintaining exceptional patient and provider experiences. Cross-Functional Collaboration: Partner with Clinical Operations, Product, Therapy, Psychiatry, Coaching, and Partner Success teams to improve workflows and align operational priorities.

Qualifications

Education: Required: Bachelor’s degree in nursing, social work, psychology OR formalized training in mental health Clinical licensure not required for this role Skills and Competencies: Healthcare Leadership: 3+ years of experience leading teams in healthcare operations, care coordination, behavioral health, care management, or related healthcare settings, with demonstrated success developing high-performing teams and driving accountability to operational goals. Operational Excellence: Proven ability to design, optimize, and scale workflows, solve complex operational challenges, and implement sustainable process improvements in a fast-paced environment. Data-Driven Decision Making: Strong analytical skills with experience using metrics, dashboards, and operational data to monitor performance, identify opportunities, and drive continuous improvement. Technology & Innovation: Experience leveraging technology, automation, AI-enabled tools, EHRs, or operational systems to improve efficiency, scalability, and team effectiveness. Cross-Functional Leadership: Demonstrated ability to partner effectively with Clinical, Operations, and Product, teams to drive change, improve processes, and successfully implement new tools and workflows. Communication & Execution: Exceptional communication, organizational, and project management skills with a strong ability to build relationships, manage competing priorities, close the loop, and drive initiatives from concept to execution. Startup or high-growth healthcare experience preferred. Experience in women's health, behavioral health, collaborative care, or multidisciplinary care models is a plus.
