Palliative Care Licensed Clinical Social Worker - Remote | WFH
Are you passionate about making a meaningful impact in end-of-life care? We're seeking a dedicated and compassionate Licensed Clinical Social Worker to join our dynamic interdisciplinary team. This remote position will play a pivotal role in providing essential support and care to patients and their families. Responsibilities... Collaborate with an Interdisciplinary Team (IDT) and the Director of Social Support Services to develop and implement comprehensive care plans for patients. Provide compassionate case management, addressing psychosocial, financial, and legal concerns of patients and families via video technology and telephone calls. Assist in accessing community resources and navigating social and healthcare systems. Mediate conflicts, conduct financial needs assessments, and guide patients through benefit navigation (Medicare, Medicaid, Disability, Social Security, Veterans Affairs). Assess spiritual and medical needs, promptly communicating these to the interdisciplinary team. Foster relationships with community resources and support and supervise social work interns. Facilitate advance care planning, provide assistance for funeral arrangements, and ensure timely documentation.
Requirements
Minimum of 2 years' experience as a Licensed Clinical Social Worker. Excellent oral and written communication skills along with positive interpersonal abilities. Proficiency in general computer skills, MS Office, and internet navigation. Preferred experience in hospice, palliative care, oncology, or home health. Palliative care certification is advantageous. Cultural competency and bilingual proficiency (English/Spanish) are highly valued. Reporting And Benefits This position is remote and reports to the Director of Social Support Services.
