Key Responsibilities • Conduct home visits and perform comprehensive social work assessments, counseling, case management, and crisis intervention. • Co-facilitate support groups, offering a safe space for patients and their caregivers. • Organize and coordinate monthly educational programs to empower and inform stakeholders. • Engage in chapter-wide events to foster community connection and outreach. Required Skills • Active New York State LSW license. • Minimum of three years of relevant experience in social work, with an emphasis on end-of-life care. • Exceptional communication and interpersonal skills. • Ability to work autonomously as well as collaboratively within a team environment. • Bilingual capabilities are advantageous. • Valid driver’s license and access to a vehicle. Qualifications • Licensed Social Worker (LSW) with a valid New York State license. • At least three years of direct experience in a similar capacity. • Demonstrated knowledge and experience in end-of-life care settings.
