Responsibilities • Strategic planning including ideation, implementation and evaluation for assigned Enhanced Primary Care (EPC) and value based care initiatives. , • Development of long term strategy of network provider engagements, healthcare cost reduction, business engagement with network physicians, and business leaders. , • Introduce, promote and monitor health value initiatives as it relates to participation and adoption by network providers. , • Direct, lead and motivate assigned members of the Physician Engagement team towards achieving distinguished performance in the Quadruple Aim as it pertains to CDPHP strategic initiatives. , • Lead teams to create information sharing strategies to grow network provider performance and satisfaction. , • Lead internal cross-functional strategy and collaboration towards achieving healthcare cost reduction and clinical quality improvements. , • Relay population and member-focused clinical information to physicians for participation in concepts, with the goal of earning physician buy-in. , • Design, create, and conduct educational meetings, as well as bring opportunities to physicians that will drive value in health care services. , • Oversee and implement strategies to manage practice performance in the EPC model and other value-based payment models. , • Create and implement analytical strategies to improve performance of the EPC sites. Requirements • Minimum five (5) years of experience working directly with physicians/practices to identify opportunities and support change required. , • Bachelor's degree or four (4) years of equivalent experience required. , • In-depth knowledge of the health care market and ability to identify the needs/concerns of health care providers required. , • Demonstrated ability to present complex information in a clear, concise manner. , • Strong communication skills, both verbal and written. , • Demonstrated project management skills, including pro-active communication of changes and progress; and completing projects on-time and within budget. , • Demonstrated ability to synthesize complex or diverse information; collect and research data; use intuition and experience to complement data; design workflows and procedures. , • Ability to interpret data and target proper markets to maximize revenues. , • Outstanding customer service skills, including demonstrated ability to manage difficult client situations and respond promptly to customer needs. , • Demonstrated ability to think strategically and adapt to changing conditions. , • Demonstrated ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. , • Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. , • Requires local travel to visit providers. Must have valid driver's license, current inspection, and appropriate insurance coverage in effect at all times.
