• ? Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), or M.B.B.S

? Residency trained and Board Certification or Board Eligibility in Family Medicine or Internal Medicine ? Active, unrestricted medical license in Florida or has Florida application in process ? Valid DEA license or the ability to obtain prior to employment Preferred Qualifications: ?
