Job title: Transition Care Coordinator/RN/LCSW/LPC/LPN- Remote in VA in Norfolk, VA at Sentara Health Company: Sentara Health Job description: Sentara Health is hiring for a Transition Care Coordinator- Remote in VA!Status: Full-Time, permanent position (40 hours)Standard working hours: 8am to 5pm EST, M-F.Location: Remote in Virginia only.Job summaryThe TCC will work in collaboration with the Care Coordinator, Interdisciplinary Care Team Meetings, and facilities to facilitate a safe and effective transition.Education:ALD - Associate's Level Degree, MLD - Master's Level Degree, BLD - Bachelor's Level Degree, N-3YR - RN-Diploma (Non-degree) preferredCertification/Licensure:One of the following licenses or education is required: RN, LPN or Bachelors degree in Health & Human Services. LCSW or LPC licensure also acceptable for certain positions REQUIREDExperience:A minimum of two (2) years of Case Management Experience or Long-Term Care Experience REQUIREDJob Experience in Nursing, Social Work or Human Service Field REQUIREDOne year working with Medicaid or CCC Plus populations, licensed in the state of VA REQUIREDHealth plan experience preferredSentara Health Plans provides health plan coverage to close to one million members in Virginia.
