Job Title: Founding Physician (Clinic Owner) — Longevity Primary Care Company: Endless Health Location: Austin, Dallas, or Houston, TX Employment Type: Full‑time — Founder/Owner (MSO partnership, not a traditional W‑2 role) Openings: 2–3 physicians only in this first cohort (highly competitive) Why this role If you’re an MD/DO who’s been sketching your ideal practice on napkins—obsessed with prevention, metabolic health, hormones, women’s or men’s health—and you light up at Attia‑style deep dives, this is your moment. Endless Health will help you launch your own longevity‑focused micro‑practice in about a week, with modern tech running the back office so you can practice medicine the new‑old way: time, depth, continuity. What you’ll do

• Own and lead a physician‑led micro‑practice serving the community you know best (e.g., Latino, South Asian, Chinese communities or others you’re culturally competent with).
• Deliver a $150/month subscription model: 20–30 minutes of clinician time per patient per month, 24‑hour response, and basic labs included via EndlessDX.
• Practice comprehensive primary care with a longevity lens: acute care + recovery + prevention, integrated with dietetics, behavioral health, and PT.
• Build and steward a 200–250 patient panel, nurturing long‑term relationships and measurable outcomes.
• Co‑create your clinic’s unique value proposition, care protocols, and content—what you want your practice to be known for.
• Uphold clinical quality, documentation, and compliance standards; leverage our EMR, intake automations, and analytics to continuously improve.

What you’ll bring

• MD or DO with active, unrestricted license in TX (or intention to obtain within a month). Board‑certified/eligible in Internal Medicine or Family Medicine (other relevant boards welcome).
• Demonstrated interest in preventive/longevity medicine (e.g., metabolic health, hormones, women’s/men’s health, cardiometabolic risk).
• Cultural competence and comfort caring for specific populations; bilingual skills (e.g., Spanish, Mandarin, Cantonese, Hindi/Urdu) are a strong plus.
• Owner’s mindset: outgoing, ambitious, kind, generous—and ready to take a smart risk with a long‑term partner.
• Ability to commit full‑time focus for the first 3 months (no other clinical or consulting work) to reach escape velocity.
• Comfort with hybrid care (in‑person + telehealth) and modern tooling (EMR, automation, patient engagement systems).
• Current DEA and malpractice coverage (or ability to obtain).

Launch timeline

• Weeks 1–2: Define niche and population; patient interviews; positioning & offer.
• Weeks 2–3: Website, waitlist, legal formation (PLLC/MSO).
• Week 3–4: Open doors—see first patients within ~1 week of forming your entity.
• Panel goals: ~100 patients to escape velocity; 200–250 within ~24 months.
• Target take‑home: $200k–$250k/year at panel maturity (depends on panel size, add‑on services, and local costs; not a guaranteed salary).
