Job Overview:

We are seeking a dedicated and empathetic Hospice Care Nurse to join our team at Amedisys, a leading home health and hospice company in the U.S. As a Hospice Care Nurse, you will have the opportunity to make a meaningful impact on the lives of patients and their families, providing high-quality care in the comfort of their own homes.

Responsibilities:

• Perform patient assessments and develop individualized plans of care
• Collaborate with the care team to provide comprehensive care and support
• Promote patient health and independence through teaching and rehabilitative measures
• Supervise LPNs and HHAs, and provide staff education and training
• Maintain accurate and timely documentation, and communicate patient progress to the clinical manager and care team
• Participate in on-call responsibilities and provide on-call services to patients and families as assigned

Requirements:

• Current RN license, specific to the state(s) you are assigned to work
• One (1+) year of clinical experience as a Registered Nurse (RN), or approval by VP Clinical
• Current CPR certification and valid driver's license, reliable transportation, and liability insurance
