Responsibilities

• Assist patients during admissions, transfers, and discharges, ensuring their personal care needs are met in accordance with their wishes and care plans.
• Review care plans daily to determine if changes have been made to the patient's daily care routine.

Qualifications

• CNA license certification prior to hiring.
• Current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred, with recent Long-Term Care/Skilled Nursing Facility or Transitional Rehab experience.
