The Licensed Practical Nurse (LPN)-Home Health is providing skilled nursing care in the patients home setting according to the nursing care plan. Utilizing physicians' orders and professional skills the Licensed Practical Nurse (LPN)-Home Health, in conjunction with the RN Case Manager, delivers care through a home health plan that meets each patient's specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, as well as agency policies and procedures. Participates in Quality Assessment and Performance Improvement (QAPI) activities. Position Specifics: Department: HSHS Home Care Schedule: Day shift; 40 hours per week, no call Location: Springfield, IL Service Area Compensation: Based on experience, plus shift differentials for nights and weekends About HSHS Home Care: Serving 37 counties across Central and Southern Illinois, HSHS Home Care provides high-quality, personalized care that helps patients remain independent and safe in their homes. Our interdisciplinary team includes nurses, aides, social workers, and therapists who collaborate closely with patients and families to ensure a continuum of compassionate care. Education Qualifications

  • Completion of State Approved LPN Training Program is required.

Experience Qualifications

  • 1 year of acute nursing experience is preferred.
  • Prior experience in home health or hospice is preferred.

Certifications, Licenses and Registrations

  • Basic Life Support (BLS) certification is required.
  • License as a Licensed Practical Nurse (LPN) in the state of practice is required.
  • Valid state driver’s license, current automobile insurance and reliable automobile is required.
